The job: hands-on care and observation
Health workers are the frontline of hands-on care in a home: helping residents get up, transfer and move, assisting with eating and personal hygiene, taking vital signs, doing simple wound care and helping with medication under a nurse's direction. They are also often the first to notice changes — a resident eating less, becoming more withdrawn, or showing early signs of a pressure sore — and must record and report these. The job is shift-based, with one shift often covering more than a dozen residents, so it is physically demanding while also involving emotional support and communication with families.
The gate: registration is required by law
The Residential Care Homes (Elderly Persons) Regulation and the Residential Care Homes (Persons with Disabilities) Regulation bar a non-nursing home operator from employing anyone who is not a registered health worker as a health worker, which makes "registered first, then employed" a firm requirement. There are two routes to registration: completing a health worker training course approved in writing by the Director of Social Welfare, or satisfying the Director, through one's education, training, professional experience and skill in health work, that one is fit to be registered. In practice, most people study an approved course first and then apply through the Residential Care Home Staff Registration System, with help from the training institute.
Renewal and career prospects
Since 16 June 2024, registered health workers who want to keep working in a home must renew their registration, submitting the application roughly three to six months before it expires; SWD is also phasing in renewal notifications for those registered before June 2024. On prospects, an SWD consultancy study has proposed a new professional rank, the "care practitioner", dedicated to nursing work in homes, which would respond to manpower needs and give health workers a step up. Longer term, with an ageing population and rising demand for residential care, formally trained and registered health workers should have more bargaining power.
